Insurance Plans
When you have an appointment at the practice, please bring with you any applicable insurance cards. If you have a xerox of your card, please bring copies of both sides. Also, please bring a photo identification to your visit.
For patients who require referrals from their primary medical doctors, these referrals must be generated prior to your visit. The staff may not be able to contact your physician to obtain a referral in a timely manner.
All co-payments must be paid in full at the time of your visit. The office no longer accepts cash. The office accepts health savings cards, checks, credit cards, debit cards and money orders. We accept Mastercard, Visa and Discover. We do not accept American Express. Effective 7/1/22, as a result of dramatically increasing costs, a 3.5% swipe fee will be applied to all card transactions. Visits not covered by insurance must be paid on the day of service.
